Neighborcare Health Homeless Youth Clinic at 45th Street has for decades provided integrated primary health care for homeless and at-risk street youth between the ages of 12 and 23.

Homeless Youth Clinic focuses on creating an environment that is safe, supportive, and accessible for youth, with services tailored to their particular needs. We proud of its twenty-one year reputation as a safe and caring place.

Our goal is always to help homeless patients into stable housing, but for those who aren’t yet there this time of year is cold and wet, but you can help!
