Immunization records

If you need a copy of your own or your child's immunization (vaccination) record, you can print a copy directly from the Washington State Immunization Record website in less than five minutes. Go to https://wa.myir.net, click on log in, and follow the instructions on how to register for an account.

If you prefer to request your immunization record through Neighborcare Health, complete a Release of Information form and return it to Neighborcare Health. Your request will be processed within five business days.

Completed forms may be mailed, faxed or returned in-person to your clinic or to the Health Information Management team at:
